The revenue system MCAG installs in your practice. Backed by a 30-day activation guarantee.
We diagnose what is actually leaking, install the Foundation that closes the leaks, and tie every engagement to tracked revenue. No campaigns. No menus. No pouring water into a leaky bucket. The Foundation is $10,000 setup and $1,500 per month. What you see below is exactly what you get.
$8.5 million in tracked client revenue across orthodontics, MedSpa, manufacturing, and adjacent service categories.

Your practice has marketing activity. Your practice is also leaking.
Most orthodontic practices we diagnose at this revenue band show the same operational signature. The owner is working harder than ever. Marketing spend is going out. New starts have flattened or dropped. The actual cause is almost never marketing. It is operational. The Foundation layer of the practice is leaking, and every dollar of acquisition spend put on top of it leaks at the same rate.
- Half your inbound calls go to voicemail and you do not have a number for how much that is costing.
- Web leads get followed up on next-day, when adult Invisalign shoppers are comparison-shopping in the next 30 minutes.
- Your dormant patient list is sitting at 200 to 500 names and nobody has touched it in 90 days.
- Your last marketing engagement felt expensive and you cannot pull a clean revenue tie-back from the CRM in 60 seconds.
If two or more of those describe your practice, you are probably in the Leaky Bucket pattern. The next step is to confirm it.
One system. Three layers. Built in this order, every time.
We do not negotiate the order. The Foundation closes the leaks. The Acquisition Engine adds fuel. The Long-Term Visibility Engine compounds. Skip a layer and the layer above it underperforms.
The Foundation.
$10,000 setup · $1,500/mo
Pilot pricing of $7,500 setup is available at our discretion for the first orthodontic practices to onboard, in exchange for documented case study participation.
The Acquisition Engine.
+$1,000/mo + ad spend
Typical practice ad spend in this category runs $2,000 to $5,000 per month.
The Long-Term Visibility Engine.
Foundation + SEO: $11,500 / $3,000/mo
Full Stack (Foundation + Acquisition + SEO): $12,000 setup, $4,000/mo plus ad spend.
Every Foundation deployment includes all of these.
No add-ons, no upsells. The Foundation is one productized install. Your team is trained on it. Day 30 you have a system that is working.
- Local Growth System, the white-labeled CRM we install in your practice, captures every lead from every source.
- AI voice receptionist for inbound coverage of missed calls.
- Outbound voice follow-up for web leads who do not book within 24 hours.
- Automated review request flow triggered after every appointment.
- Google Business Profile optimization for the local pack.
- Dormant patient reactivation sequence (90-day, 6-month, 12-month touch cadence).
- Birthday and anniversary campaigns for the existing patient list.
- Referral request automation tied to consult-finished events.
- Missed call text-back fallback so the voicemail-only fallback never costs a consult.
- Lead nurture sequences for unconverted leads from any source.
- Retargeting pixels and custom audiences ready for Acquisition Engine activation.
Optiskyn. The methodology in numbers.
Optiskyn is an aesthetic wellness studio operating in the same revenue zone as our target ortho practice ($800K to $5M owner-operator). We installed the Foundation first, then the Acquisition Engine. The numbers are what we got.
“1,540% return on $664.92 in ad spend. 19 booked phone consults from 29 leads. 100% phone consult show rate. $10,901.75 in tracked campaign value. Most recently reported more revenue in Q1 2026 than in all of 2025 combined.”
The Optiskyn engagement
Optiskyn is a medspa, not an orthodontic practice, and we say so plainly. The methodology and the prescription are the same. Foundation first, Acquisition Engine second.
Why we share it on this page: an $800K to $5M owner-operator practice with a Leaky Bucket pattern responds to the same install in the same way. The case fee math changes (Optiskyn membership vs. ortho treatment plan) but the system math does not.
If the system is not alive at day 30, your first month’s recurring fee is refunded.
Within 30 days of activation, the Foundation is live in your practice and producing measurable activity. Calls covered. Reviews coming in. Dormant patients hearing from you again. CRM capturing leads from at least one active source. If that does not happen, we refund your first month’s recurring fee.
We do not guarantee a specific number of new patients in 30 days, because the market’s response is partly outside our control. The Foundation is what we own. The market response is what the Foundation earns.
We are clear about who we decline.
Saying yes to the wrong client costs us more than saying no. Both sides should know the fit before the call:
- Owners who want to skip the diagnosis and hire a body shop.
- Owners shopping for the cheapest agency on the call. We are not it.
- Owners who want a tool license without a system installed underneath.
- Practices below $800K in annual revenue. The Foundation is built for owner-operators with revenue to defend, not pre-revenue startups.
We work with orthodontic owner-operators in the $800K to $5M revenue band who want to be shown what is actually broken and walked through the fix in plain language. If that is you, the next step is below.
Six things owners ask before booking.
How long until I see new patients from this?
The Foundation produces measurable activity inside 30 days, which is what the Activation Guarantee covers. Tracked new starts depend on which layers you turn on and your local market. The Optiskyn case study below is the cleanest analog: $664.92 in ad spend produced 19 booked phone consults, 100% show rate, $10,901.75 in tracked campaign value. We do not promise that exact return for your practice. We do promise the system is alive and working at day 30.
I already have a marketing agency. Why would I switch?
If your existing agency can pull tracked revenue per dollar of ad spend out of your CRM in 60 seconds, you probably do not need to switch. If they cannot, the question is not switching. The question is whether you are paying them to fill a system that does not convert and retain what arrives. We diagnose that on the Discovery Call.
Do I have to run paid ads?
No. The Foundation is the leak-fixing layer and is required for every client. The Acquisition Engine (paid) is optional and we usually do not recommend it until the Foundation is producing measurable activity. Many clients see meaningful revenue lift from just the Foundation, because the dormant patient list and missed calls are usually the largest unworked revenue in the practice.
What is the Trojan Horse Smile Funnel?
The name we use for the Acquisition Engine when applied to ortho practices. A free in-office consultation is the on-ramp; paid acquisition (Meta primary, Google secondary) drives the booking; the Foundation does the conversion and retention. We did not invent the in-office-consultation pattern. We productized the version that converts.
How is this different from just buying a CRM license?
You can rent the same CRM you rent today for under $100 a month. What you cannot rent is a system that uses it. We install the operational backbone (call coverage, lead follow-up, reactivation, review velocity, retargeting) on top of the CRM, train your team to operate it, and back it with the 30-Day Activation Guarantee. The CRM is one of the tools. The system is the asset.
What does the 30-Day Activation Guarantee actually cover?
Within 30 days of activation, the Foundation is live and producing measurable activity (calls covered, reviews coming in, dormant patients hearing from you again, CRM capturing leads from at least one active source). If that does not happen, we refund your first month's recurring fee. We do not guarantee a specific number of new patients in 30 days because the market response is partly outside our control. We guarantee the system is alive.
Book the call. Walk out with a named pattern, an audit report, and a plain-language plan.
Structured diagnostic conversation, not a sales pitch. You leave with a branded R6 Audit Report inside one business day. You keep the report whether or not you engage MCAG. We do not chase.
Take the Five Patterns Self-Diagnosis. Three minutes. Owner-self-reported. No email gate to see your result. The recommendation is yours whether you book a call or not.
